Form Instructions: Start by logging into the database if you already have an account. As you complete the form, new fields will open as needed. For the most secure membership payment processing, Am Tikvah advises members to set up your own ShulCloud Database account*. Then you can make payments via ACH or Credit Card. You can set up recurring monthly, quarterly, or one-time payments by choosing the Pay Online option. Please return this form to the office by or before June 30, 2025. (This will help us accurately assess our budget and plans for the coming year.) Check all boxes indicating your preferences. (You will receive a confirmation email after you submit this form to the office.) We recommend you pay online through our secure database. If you choose to receive paper bills, please indicate you preference below and indicate your preferred billing frequency. Our fiscal year runs from July 1st - June 30th. Membership pledges should be fulfilled by or before June 30, 2026.
